1. MEMBERSHIP DETAILS

Program: Muay Thai & Kickboxing – Teens & Adults

Promotional Membership Rate: $149 per month

Training Access: A minimum of 3 training days per week will be offered.

Potential training days may include Monday, Tuesday, Wednesday, Thursday and/or Friday.

Exact training days and times may change as the program develops.

North Durham Taekwondo Academy intends to provide at least three training days each week. The program may operate on any combination of the days listed above.

Training times, instructors, class formats and individual training days may be adjusted based on instructor availability, holidays, attendance, facility needs or program operations.

2. PAYMENT & MONTHLY BILLING

Membership fees are billed monthly.

The date of enrollment will also become the member’s recurring monthly billing date. Payments will normally be processed on or about the same calendar day each month while the membership remains active.

Payment may be made by:

• Pre-Authorized Debit (PAD)
• Credit Card

Pre-Authorized Debit authorization may be completed separately through North Durham Taekwondo Academy’s electronic payment form.

Credit card information may also be provided separately through the Academy’s payment system.

Credit card payments may be subject to a processing fee of approximately 1.9% to 2.9%, depending on the payment processor and where permitted. Any applicable processing fee will be disclosed before the charge is processed.

By accepting these Terms & Conditions and completing enrollment, the member authorizes North Durham Taekwondo Academy to collect the agreed recurring membership fees using the payment method provided while the membership remains active.

Membership fees provide access to the program and are not reduced because of missed classes or personal absences, provided the Academy continues to make the program reasonably available.

3. TRAINING, SAFETY & PARTICIPATION

Muay Thai, kickboxing and related fitness activities involve physical exertion and may include:

• Striking drills
• Pad work
• Bag work
• Partner drills
• Conditioning
• Controlled contact or sparring when approved by an instructor

Participants understand that these activities involve inherent risks, including but not limited to strains, sprains, bruising, falls, accidental contact and other injuries.

Participants agree to:

• Follow instructor directions
• Use equipment appropriately
• Train within their own ability
• Immediately stop and notify an instructor if feeling unwell, injured or unsafe
• Follow all reasonable safety instructions

Each participant is responsible for determining whether they are physically able to participate and for obtaining medical advice when appropriate.

4. ASSUMPTION OF RISK, RELEASE & INDEMNITY

By participating in the Muay Thai & Kickboxing program, the participant voluntarily chooses to participate and accepts the inherent risks associated with Muay Thai, kickboxing and related training.

To the fullest extent permitted by law, the participant releases and holds harmless North Durham Taekwondo Academy, its owners, instructors, employees, contractors and representatives from claims arising from the ordinary risks of participation, except for rights or liabilities that cannot legally be waived.

The participant agrees to be responsible for losses or claims caused by their own intentional misconduct or failure to follow reasonable safety instructions.

5. PHOTO, VIDEO & SOCIAL MEDIA AUTHORIZATION

Classes, events and training sessions may be photographed or recorded.

By accepting these Terms & Conditions and completing enrollment, the participant authorizes North Durham Taekwondo Academy to capture and use photographs, video and/or audio in which the participant may appear for academy-related:

• Social media
• Website content
• Advertising and promotional materials
• Instructional purposes
• Academy communications

Such use may occur without additional compensation.

Participants understand that training takes place in a shared group environment and that incidental appearances in group photographs or videos may occur during normal academy activities.

For participants under 18 years of age, this authorization is provided by the parent or legal guardian completing and signing the enrollment.

6. ACADEMY RULES & MEMBER RESPONSIBILITIES

Participants agree to:

• Treat instructors, staff and other participants respectfully
• Follow reasonable safety and conduct rules
• Use protective equipment when required
• Not participate in sparring or contact drills unless authorized by an instructor
• Notify the Academy if contact or payment information changes
• Follow instructor directions regarding training and equipment

Participants are responsible for keeping their personal belongings secure.

North Durham Taekwondo Academy is not responsible for lost, stolen or damaged personal property except where required by law.

Training schedules, instructors and class formats may be adjusted as the program develops while maintaining at least three training days per week whenever reasonably possible.

7. TEEN PARTICIPANTS

This program is intended primarily for adults and teens.

If the participant is under 18 years of age, a parent or legal guardian must complete and sign the enrollment on the participant’s behalf.

The parent or legal guardian confirms that they have the authority to provide the permissions, acknowledgments and authorizations contained in these Terms & Conditions for the minor participant.

8. ACCEPTANCE OF TERMS

By completing the enrollment form and signing electronically, the participant — or the parent/legal guardian for a participant under 18 — confirms that they:

• Have read and understood these Terms & Conditions
• Have had the opportunity to ask questions
• Agree to the membership and payment terms
• Acknowledge the physical nature and risks of participation
• Accept the assumption of risk and release provisions
• Agree to the photo, video and media authorization
• Agree to follow the Academy’s safety and conduct requirements

The participant agrees that an electronic signature has the same effect as a handwritten signature.
